If you have commercial insurance and meet other eligibility criteria, you may pay as little as $0.

To be eligible, you must:

• Have commercial insurance and coverage for your treatment
• Be a resident of the US or a US territory
• NOT be a beneficiary of Medicare, Medicaid, VA/DoD (TRICARE) programs, the Indian Health Service, or other federal or state healthcare programs, even if patient pays for prescription in cash or if the patient chooses not to use their insurance coverage

If you have Medicare but not Medicare Part D

• (ON)Path may be able to help you

If you have Medicare and are low income

• You may be eligible to receive support through the Medicare Low Income Subsidy Program (LIS). An (ON)Path Patient Access Advocate can help you understand if you are eligible

The Patient Assistance Program may be available for you if you are uninsured or underinsured and meet other criteria.

If you meet certain medical and financial requirements, you may be eligible for the Patient Assistance Program. To apply for this program you will need to consent to income verification, which may include a soft credit check, and other steps.
